- Vaccination Status
Why are we collecting it?
The purpose of collecting this information is to review the trusts vaccine compliance in line with our national objective set by NHSE and the UK Health Security Agency (UKHSA).
What is our legal basis for collecting this information?
We will be processing your information under the following conditions:
- Article 6 (1) (a) Explicit Consent
- Article 9 (1) (a) Explicit Consent
- Article 9 (2) (h) Management of Healthcare services
Where will your information be stored?
Your information will be stored in a database.
Is the information shared?
This information will be aggregated and shared with NHSE and the Trusts Executive Committee.
Is the information transferred overseas?
Your information will not be shared overseas.
How long will we hold the information?
This information will be held in line with retention periods set out by law and will be held for no longer than necessary. As a Trust we will hold this information in line with healthcare data retentions set out by the NHS Records Management: Code of Practice unless any new changes to legislation state otherwise.
What rights do you have around your information?
You have several rights under the data protection law:
